To be eligible for services, you must be:

• Home-bound
• Disabled and in need of mobility support/ambulation assistance
• Prescribed by your physician to receive home health care services
• Medicare/Medicaid recipient

Our services can be provided on a regular schedule or on an intermittent basis. Depending on your need for health care services and personal assistance, our office will coordinate care with you - the patient - your family members and your doctor.
